Congratulations on your successful mentorship! Below is the Assessment Packet that you will complete with your Artist Mentor at your Assessment Visit. We will need everything on the list in order to complete your mentorship.
Your Assessment Packet should be completed at your Assessment Visit and consists of the following elements:
A.R.T. (Autonomy Rubric for Teachers) – Teacher (except Arts Foundations) and Artist Mentor

Photos
Visual Arts: Five digital photos of student artwork that meet criteria should be sent to Arts Impact using one of the following options: this link: photos; or email the photos; or text the photos-include your name-(253-569-9247).
Performing Arts: Upload the video to Dropbox.com. (If sending multiple videos, please select up to 5 videos that show criteria to upload.)
Classroom Assessment Worksheet – Teacher and Artist Mentor
This is the Classroom Assessment Worksheet from your lesson. Both the teacher and the Artist Mentor should turn this in; be sure to indicate who is completing the assessment in the upper right corner of the worksheet. If you can save the document to your computer in any format (Word, .pdf., .jpg, etc), use this link to send it: Assessment. Your Lesson Plan
Please send us the final version of your lesson plan written with your Artist Mentor during your mentorship. You may save the any changes to your electronic version and send it using this link: lesson plan. You may also scan & email the lesson plan; or fax it to (425-917-7810); or take a photo of the pages with the handwritten changes and either upload the photos to Dropbox.com or text the photos-include your name-(253-569-9247).
